Seems like you've more or less understood the aim of the task.
Put your name, class, date, and title on the document.
I don't see how that poem can be called a limerick...
If you wrote the poem, how come you keep calling the author "the writer"; also you've called it "poem example", is this your writing?  Some grammar issues are obscuring your meaning.
